    We would definitely like to use this add-on, but the documentation you provide doesnt seem to answer questions I have. - this form if embedded in my website and as a link "Open Support Ticket" would create Support Request in Service Desk as a JIRA User ..? - Will I be able to map or use dropdown to pick type of Service Request to be created from this form. - If possible could you please upload a small video explaining the functionality of this plugin end to end. We would definitely want to use this plugin and are looking forward for response from you.

    Hello Raghuram Pulijala,

    thank you for your feedback. To answer your questions:

    1. You have to enter a Service Desk Customer in the settings for every xform, who will be the creator of the ticket. In the settings, you will get a link to an iframe, which can be included on any website.

    2. You can create one xform for for every Request Type. At the moment you would have to create the DropDown by yourself and update the iframe with the xform. It's not possible to chose the request type at the moment.

    3. We are going to update xform by time and improve the documentation as well.

    1. Log into your JIRA instance as an admin.
    2. Click the admin dropdown and choose Add-ons. The Manage add-ons screen loads.
    3. Click Find new add-ons from the left-hand side of the page.
    4. Locate Service Desk xForm via search. Results include add-on versions compatible with your JIRA instance.
    5. Click Install to download and install your add-on.
    6. You're all set! Click Close in the Installed and ready to go dialog.
