    I can really say that the team I was in contact with(Rafael and Divya) regarding a function I interpreted to work a certain way, really went the extra mile to nail down my issue and also help me fix it. Now, Forms for Confluence works flawless. This is what customer support really should be like - communication both ways.     After a full trial we decided to buy this plugin. We use it in different way: just as form to notify someone and as form to fill some information in database. Notify feature works perfectly by design but there are few things to improve: ability to customize mail subject with dynamic content. Storing, retrieving, manipulating and displaying information from database (from form in terms of plugin) it's a subjects to improve and I hope to see changes in next versions of plugin. In general I'm satisfying with plugin and with support team: they react quickly and accept bug report as well as feature requests.

    Although I've had my own trials with this plugin, I want to commend Rafael and Divya for working directly with me to address the issues I encountered with the Forms for Confluence plugin. I was able to provide the right kind of information that allowed Rafael to find and quickly correct the plugin issues. The Forms plugin is a nitch plugin, but one that some of my user rely upon for some of their business processes integrated into our Confluence instance. And working with Rafael allowed me to help identify some confusing logging output. But it was the speed of the corrections that was most helpful. I have bug reports on other plugins since we just upgraded to 5.8.18, and too many of them just drag on from week to week. But that was not my experience here. As a matter of fact, I had two snapshots to correct the problem before I could get an environment in a position to test them. Even my complaints about their documentation deficiencies were addressed faster than I would have expected. This is the way its supposed to work, and right now, I couldn't be happier about my experience. So, well done Divya and Rafael! And thank you to you both for the great support. Steve Hadfield

    The latest version of Form Mail for Confluence 4.1.x fails to install on Confluence 5.6.6 and the support process is torturous, adaptavist is on London time and poorly serves the West coast of America. Additionally opening a ticket with them for a licensed product is not really possible in the system they have setup. This product should be avoided at all costs. Do not purchase a license. Do not use this product.

    How does server add-on pricing work?

    Server products and add-ons are hosted on your servers. Licenses are perpetual and the purchase price include 12 months of maintenance (support and version updates).

    You can renew maintenance after 12 months at 50% of the current purchase price. You can upgrade the tier of your host product and add-on licenses at any time. Upgrade prices are calculated based on Atlassian's formula (view example).

    If add-on pricing changes after your initial purchase, there's a 60-day grandfathering period during which you can renew based on the old pricing.

    How do I determine my server pricing tier?

    The add-on tier should match the licensed user tier of the Atlassian host application. For example, if you have a Confluence Server license for 25 users, you should purchase the 25-user tier for add-ons. Even if fewer users want to use the add-on than your host application license, the two licenses should match exactly.

    Do you offer academic, community, or open-source licenses?

    For server add-ons, purchase and renewal is half-price if you have an academic license for your Atlassian host application. Server add-ons are always free for community and open-source licenses. Cloud add-ons do not have discounted or free licenses.

    Can I extend my free trial?

    For server add-ons, you can extend your add-on trial up to 5 times - in other words, for up to six months. Extend your trial by generating a new evaluation license key from Atlassian Marketplace. Click Try it free and you'll be directed to generate a new license. Paste this license key into the add-on listing in UPM from your Atlassian host application, and you're all set.

    How can I buy add-ons for my legacy JIRA Server or Confluence Server license?

    If you own a legacy JIRA Server Unlimited (100+ users) or Confluence Server Unlimited (2000+ users) license purchased in 2012 or earlier, legacy add-on pricing is no longer available. You have two options for add-on purchasing:

    • Purchase the add-on at the non-legacy Unlimited (10000+ users) tier.
    • Renew your JIRA or Confluence license at a non-legacy tier, then purchase the add-on at the same tier.

    Statistics and Anonymous submissions


    • Statistics - Visualise statistics for your responses. Drill down and understand trends on your responses.
    • Anonymous submission - Now you can allow anonymous submissions which will not reveal whom submitted a response.
    • Maximum length - Added maximum length for Input Field and Textarea field.
    • Go to page condition - Go to page condition permits to jump to a particular form page if the condition is matched. Very useful if the user does not need to fill the details of particular pages during the process
    • Bug fixing.


    1. Log into your Confluence instance as an admin.
    2. Click the admin dropdown and choose Atlassian Marketplace. The Manage add-ons screen loads.
    3. Click Find new add-ons from the left-hand side of the page.
    4. Locate Forms for Confluence via search. The appropriate add-on version appears in the search results.
    5. Click Try free to begin a new trial or Buy now to purchase a license for Forms for Confluence. You're prompted to log into MyAtlassian. Forms for Confluence begins to download.
    6. Enter your information and click Generate license when redirected to MyAtlassian.
    7. Click Apply license. If you're using an older version of UPM, you can copy and paste the license into your Confluence instance.
