Mark any Jira issue as a tracked action item with an enforced owner and due date, and tag its source (retro, sprint, manual) so it's queryable.
Overdue items automatically resurface: a scheduled nudge comments on the issue and notifies the owner, and a carry-over count tracks how often a task has slipped. A "carried over" view shows everything still open before the next retro.
