Summary: Fix setup-state accuracy, access-revoked consistency, and Jira-error clarity
Details: Fixed setup-state accuracy, access-revoked consistency, and Jira-rejection clarity.
Setup screen: activation state now reads correctly, so it no longer gets stuck showing a scan as running after it finished, or shows monitoring as unconfigured when active. Settings in effect before activation now carry through, and a setting change starts a fresh scan under the new config.
Access lost mid-session: the queue, a finding's detail dialog, and setup now remove unusable controls entirely (pager, loaded cards, the detail form) rather than disabling them, including when access is lost mid-dialog.
Jira rejections: a definite rejection (e.g. 403) now surfaces immediately and plainly, instead of a vague check-manually message, now reserved for genuinely unresolved outcomes like a timeout.
Comment button now updates live as you type. Added a divider between the assign and comment sections. Confirmed resolving a blocker refreshes downstream dependency impact.
Test suite: 251/251 pass.
