Every approval binds to a page version. When someone edits an approved page the status flips to Stale in the byline, any lock the app applied is released, and the audit trail records what changed and when.

Most approval apps record a decision and stop there. Someone edits the page a week later and the badge still says Approved.
Approve & Lock binds every approval to the exact page version it was given for. When the page changes the status flips to Stale in the byline where readers actually look, any lock the app applied is released, and the audit trail records what changed and at which version.
Version-bound approvals: approved at v12 means approved at v12, not approved forever.

Requesting approval, approving and rejecting each check that the acting user holds edit permission on that page, and are refused if not.
Adds the read:confluence-content.permission scope, which is what the check requires. No other behaviour changes: anyone who could already act on a page still can.
