With the Question Fields app, you can use custom fields as survey questions

You can define a question. Answers can be obtained from text field, select list (multiple choice) or radio button.

You can define a question. Answers can be obtained from select list (single choice) or checkbox.
View questions and answers on issue panel
Display questions and answers on issue panel. Also, Information on who answered the question and when it was answered are displayed for each field.

Feature Highlights
- Question Fields app provides these custom field types; text, select list(Multiple/Single Choice), radio button and checkbox
- You can add question or information under each custom field
- Question Fields app stores selected values and shows them in question fields issue view panel
