Smart Forms for Jira (Works in JSM, JWM, JPD & Confluence)

[Edit] Very responsive support and fast turnaround time on fixing reported bugs. I find the forms are minimally better than the native Jira forms: inability to use dynamic fields like user pickers, tickets, etc, severely limits its usefulness.

Smart Forms has allowed us to share forms externally to a lot of different people and orgs. Although there has been some downtime, the support team at SaaSJet is always very quick and responsive.
