Form macro builder for Confluence

It seemed so.. but actually no. Disappointing. Fairly easy implementation, easy to set up and create forms. The intuitive setup of Form Builder in Confluence holds promise, but I've encountered a critical issue: responses are being duplicated across pages and in the original template, impacting data accuracy. This problem, coupled with the manufacturer's lack of support, has affected our experience. I hope for improvements to address these issues in the future and provide a more reliable solution. Doesn't work. No documentation to support and the dropdown for Apps is empty.

This is very simple but would meet my requirements if the range component actually worked. Right now 06-02-2022 it does not work.

the range component is not working properly
