You should add custom field to your create issue screen and check project permissions.
Be careful: you should set Captcha Custom Field as required in custom field scheme.

Captcha Custom Field works with a project with anonymous access. Anonymous users should enter secret captcha word when they try to create issue. For configure custom field you should

- Create Issue permission should have "Group: Any"
- Add Captcha Custom Field to create issue screen
- Set Captcha custom field as required and reporter field as optional in a Custom field scheme

Installation
- Log into your Jira instance as an admin.
- Click the admin dropdown and choose Add-ons. The Manage add-ons screen loads.
- Click Find new apps or Find new add-ons from the left-hand side of the page.
- Locate Captcha Custom Field for Jira via search. Results include app versions compatible with your Jira instance.
- Click Install to download and install your app.
- You're all set! Click Close in the Installed and ready to go dialog.
