    We love ProForma!! We've been using ProForma forms on our Jira Service Desk since day 1 and they have been incredible. Makes life so easy for our customers, our internal users and me as an admin. The product is fantastic and the support is out of this world, they are always super responsive and willing to help out with any issues. Also very willing to accept new feature recommendations and every release has relevant and useful features. All in all, ProForma is the #1 form app for Jira Cloud backed by fantastic customer service. You will not be disappointed!

    We went with ProForma Forms as part of our Jira Service Management implementation, but are finding uses in our Jira Software environment as well. The UI editor is easy to use and forms entry can be invoked from portals or the Jira Create Issue function. Appreciate the fact that there is no need to create a custom field for every bit of data that needs to be collected. The Data Connection feature is great for dynamically populating dropdown lists with SQL data. Recently submitted a medium level incident report and the response and resolution provided was outstanding...issue resolved in less than one hour!

    Proforma is a great tool with a number of predefined templates. The new features like "Preview and Test the forms" before publishing are really of great use but I would appreciate if Thinktilt can provide more features like having : Preview button(in the portal along with submit button)- which helps the User to view the values entered by them in the form and then gives them the option to either go "Back" and edit the form contents or "Submit" the form.

    Proforma is a nice tool to allow for extra fields to be captured without adding these fields to Jira itself as an admin. Very convenient and hassle free. Thus far I would recommend the tool!

    The joys of using this wonderful paid plugin are overshadowed by its lack of delivering on one main feature, Linked Fields. Linking fields from a Proforma form to a Jira field works 25% of the time. Even worse, after successfully setting up your form, linked fields become unlinked without reason. To fix it, you must go through a bunch of hoops to relink every field; then repeat with each form. Recently, it's gotten even worse by showing me Jira fields from other projects _and not from the project I'm working in_. I'm getting tired of hearing from support team that "the new hire onboarding form is broken again". "Fix Proforma Forms Linked Fields" is now a daily recurring task on my calendar.

    ProForma was exactly what we were looking for when revamping our incident handling process and looking for a simplified way to capture information and guide users through a process, while allowing us to store all the captured information in a Jira ticket for workflows and reporting purposes. With its Confluence-like WYSIWYG editor, ProForma is a breeze to use and does exactly what it says on the box. Highly recommended!

    Proforma has been an excellent tool for reducing the number of custom fields required to get basic information from our customers. One thing I do wish it could do is allow for limiting choices on a linked field. For example, we have a custom field that lists all of the company departments and some of our forms use this field. However, it would be awesome if we could limit some forms so that only certain departments are available to be chosen while still letting that field link to a jira custom field.

    ProForma has been a crucial piece of our Jira platform since day one. The only reason we wanted it in the first place was as a solution for our legacy business forms that needed to migrate into our new service center, but that had fields too specific to use in more than one place. ProForma probably saved us from creating and managing 100+ fields right off the bat. We have leaned heavily on ProForma, most recently as a solution to bring our Facilities team into Jira alongside IT and Marketing. It honestly is what made it possible to fully migrate from the legacy Facilities system with all the various preventative maintenance checklists and forms that need to be done. I appreciate how easy it is to access form data in automation and Scriptrunner, especially since the newer form builder was released with updated JSON format. The support is also great. We have had very few issues stemming from ProForma, and when we have, ThinkTilt support is very responsive and helpful and has resolved issues quickly.

    ProForma has been in our toolbox for a long time now, and it's incredibly useful for capturing issue information without the need for additional custom fields. We use it to capture information, and combine it with other tools (like Automation for Jira) to help triage issues. Their support is also incredibly responsive and knowledgable when resolving issues.

    This is one of the Plugins I totally rely on when designing different forms for our most complex requirements.     How does cloud app pricing work?

    Cloud apps are sold as a monthly or annual subscription. You are eligible for support and automatic version updates as long as your subscription is active.

    When your subscription renews each month, you are automatically billed for apps based on the number of users in your instance.

    If app pricing changes after your initial purchase, there's a 60-day grandfathering period during which you can renew based on the old pricing.

    How do I determine my cloud pricing?

    Apps are billed based on the number of users in your Atlassian product. Jira Cloud apps are priced based on the maximum users of the Jira products on your instance. For example, if you have Jira Software (50 users) and Jira Service Management (10 agents) on the same instance, you pay the 50-user price for apps.Note: While this app has features specific to Jira Service Management, the app is technically available across the whole Jira instance. Therefore the above guidelines for licensing across maximum users still apply.

    The pricing structure for cloud apps is as follows:

    • Monthly subscriptions with up to 10 Atlassian product users are billed at a flat rate price.
    • Monthly subscriptions with more than 10 users are billed per user, starting at the first user.
    • Monthly subscriptions with more than 100 users are billed per user. The table above provides example pricing at various user levels.

    Annual subscriptions may offer a discount depending on the number of users purchased.

    Do you offer academic, community, or open-source licenses for cloud apps?

    Yes. Academic, community, and open source licenses are available to qualifying organizations. See our Purchasing & Licensing FAQ for more information.

    Can I extend my free trial?

    For cloud apps, you cannot extend your free evaluation period. All cloud apps are immediately subscribed by a user, and we provide a free evaluation period. This is a minimum of 30 days and ends on the second billing cycle after you first subscribe to the app.

    Installation

    1. Log into your Jira instance as an admin.
    2. Click the admin dropdown and choose Atlassian Marketplace. The Manage add-ons screen loads.
    3. Click Find new apps or Find new add-ons from the left-hand side of the page.
    4. Locate ProForma: Forms & Checklist for Jira via search. The appropriate app version appears in the search results.
    5. Click Try free to begin a new trial or Buy now to purchase a license for ProForma: Forms & Checklist for Jira. You're prompted to log into MyAtlassian. ProForma: Forms & Checklist for Jira begins to download.
    6. Enter your information and click Generate license when redirected to MyAtlassian.
    7. Click Apply license. If you're using an older version of UPM, you can copy and paste the license into your Jira instance.

    5. You're all set! Click Close in the Installed and ready to go dialog.
    1. Log into your Jira instance as an admin.
    2. Click the admin dropdown and choose Atlassian Marketplace. The Manage add-ons screen loads.
    3. Click Find new apps or Find new add-ons from the left-hand side of the page.
    4. Locate ProForma: Forms & Checklist for Jira via search. The appropriate app version appears in the search results.
    5. Click Try free to begin a new trial or Buy now to purchase a license for ProForma: Forms & Checklist for Jira. You're prompted to log into MyAtlassian. ProForma: Forms & Checklist for Jira begins to download.
    6. Enter your information and click Generate license when redirected to MyAtlassian.
    7. Click Apply license. If you're using an older version of UPM, you can copy and paste the license into your Jira instance.
