Does not accept Conf cloud API key. Have to uninstall.

Unfortunately Confluence Cloud does not provide any methods in the API that would be useful for this app.
So instead, the app works via Atlassian Organisation API for managed users. This process to connect to this API is documented in Getting Started with User Management for Confluence Cloud page.
Please note, you do not have to purchase Atlassian Guard for this, but to perform some operations (e.g. deactivation) only available to managed users you do have to verify your domain and claim your managed users.
